I was frustrated with the "Rate your pain on a scale of 1-10" question with no regard to duration or type of pain....so...I made up the following pain chart and handed it into my Pain management Dr. just before my last epidural. Enjoy. :)
_
*Susan’s Pain Level Chart *_

*Pain Level 10: Non Stop Excruciating Pain*
The kind you pass out from. You should be taken directly to a hospital if this occurs.
* Female Example: Non Stop Hard Childbirth Labor.
Male Example: Having your skin peeled off.

*Pain Level 9: Intermittent Excruciating Pain*
You could still pass out, but if you don’t …call us.
* Female Example: Hard Childbirth Labor
Male Example: Being kicked in the privates every two minutes

*Pain Level 8: Non Stop Severe Pain*
You won’t pass out but you can’t function either.
* Female Example: Non Stop Transitional Labor (Your focal point is working
but you can’t do anything else except to concentrate away the pain)
Male Example: Nuts in a vise

*Pain Level 7: Intermittent Severe Pain*
You can function in spurts. When the pain gets to be too much you must take breaks and wait for the pain to subside before being able to continue with what you were doing.
* Female Example: Transitional labor
Male Example: Shopping with the Misses

*Pain Level 6: Non Stop Moderate Pain*
You can function but the non-stop ache is limiting, uncomfortable, distracting, and depressing.
Female Example: Non Stop Period
Male Example: Infected Toothache

*Pain Level 5: Intermittent Moderate Pain*
Really? Take a couple of those over the counter pain relievers and don’t call us until it’s worse.
Female Example: Period
Male Example: You just hammered your own thumb

*Pain Level 4: Non Stop Mild Pain*
Suck it up Buttercup and just deal with it…or else find a good hiding place to cry it off in because NOBODY wants to see you like that.
Female Example: Hang nail or irritation due to certain people you work for who will remain nameless
Male Example: Splinter or watching your team lose

*Pain Level 3: Intermittent Mild Pain. *
DO NOT CALL US just to escape a visit with your In-Laws!

*Pain Level 2 and 1: This is what we call NORMAL. If you can’t handle it…we can’t help you!*

*Pain Level 0: Pain Free. If you’re not dead, it’s not free. :)*

*If you never experienced childbirth without the help pain medication just refer to the male examples.

*SENT AN EMAIL TO CLAVEL:
*

Dr Clavel,Wanted to set up a call with you. I am pretty set with you, but I have some questions.

1. What method of anterior fusion do you use(instrumentation)? If one had unstable spondylolisthesis, isnt an anterior fusion usually ruled out?

2. Why did you recommend me an ADR at L4/L5? Do you feel my disc at L4/L5 is degenerated beyond a normal limit and causing many of my symptoms?

3. With the anterior approach to fusion, wouldn't the pars interarticularis/ lamina fracture still be there?

4. The purpose of the fusion in this case is to stabilize and decompress the L5/S1 vertebral bodies?

5. When is an estimated procedure date if we decide to do one?

6. What are your preferred methods of payment?

Thank You

Dr. Mkrdician recommends a "Decompression laminoforaminotomy with bony fusion."

I want to get a second opinion but not sure who to go to. I have researched many MD's. Dr. Fessler out of Northwestern, is *not* in my HMO IPA. I would have to go to a doc at Evanston, Skokie or one of several other hospitals in Northern Illinois.

I am also looking to find someone who has had this diagnosis or this same procedure. I want to know more about it.

Thanks

Hello everyone, I'm glad to "meet" you!

I'm a 50 (51 next month) Soldier that was diagnosed recently with 2 herniated discs, 5 bulging discs, and facing surgery as soon as they can get me in.

I've tried chiropractor (didn't work), Physical Therapy (actually made things worse), undergone 2 rounds of cortizone injections which the only "help" they gave was help to make things worse! (first round - July 30 was at L2-3 and L4-5; second round - Aug 15 was L5-1). When I called the doctor who did the cortizone to see if they could suggest something to relieve my pain after the shot, I received the standard answer "it can take up to 10 days to work..." obviously that didn't help!

On a 10 point scale, I "live" at a 7 and most of the time by the end of the day, I'm pegging out at 9 or 10--sometimes 15--which is when I want to break things. Percoset and muscle relaxers keep me sane, but recently they are being challenged.

Now I'm facing surgery. I'm supposed to meet with surgeon on Thursday to learn what he plans to do to help. What questions do I need to ask, and what can I expect? Usually when I have an exam I am at a level 20 before I get out of the office.

Thanks for letting me vent!

Thanks for everyone's great input. I helped me decide on Dr. Bertagnoli for ADR. I have a question that sounds a little paranoid.

I am dealing transferring the hospital and doctor's fee into an account in New York. This is a lot of money and I just want to be sure I'm dealing with the correct people. This is a lot of money and I'm feeling cautious.

Could those of you who have seen Dr. Bertagnoli confirm this for me?

Thanks,

Hi Everyone 5 and half months post op PLIF with Dr P in Brazil and unfortnately Im still no better weight bearing or standing than before the operation.Pain is low as long as I dont stand up for more than 5 or 6 minutes.I can do hydrotheraphy twice a week and very mild pilates and thats about it.I have significant SIJ pain which is worse after the op. It feels like Im compressing on some nerve that releases as soon as I lie back Down.Does anyone know whether a standing MRI would show anythng or would there be too much metal

Hello Everyone, I hope this message finds you well. I wanted to shed some light on recent issues ...I just went for my second Mri 7-21-11 , at the c3-c4 level there is a grade 1 retrolisthesis with degenerative disc bulging & unicinate joint hypertrophy resulting in moderate bilateral neural foraminal stenosis & severe spinal cord compression. c4-c5 level severe right & moderate left neural foraminal stenosis with severe spinal cord compression.
I just went for epidural injections to reduce pain in neck from 6/7/11 to 6/21/11.
The first spine surgeon I seen Dr. john r. ehteshami wants to fuse 5 disc's from the back. The second spine surgeon Dr. Duane Pitt I seen wants to fuse c3,c4,c5 from front then a week later go in the back and fuse c3 thru c6.He said he would do it that way cause when you get more thtan 3 disc's fused , that there is always on disk that does not fuse. So I don't know who to go with . i am waiting to see a 3rd spine surgeon. can anyone help me out

Hi All
Finally biting the bullet and getting cervical ADR with the m6. Or at least i think. I read that Dr. Yue at Yale prefers it and Dr Boeree said it was the preferred ADR in Europe. I am completely confused as to what levels. I have been told discograms are not so accurate due to localized edema it can affect adjacent levels and cause a sort of false positive. But what do you all think?

I have a c6-7 herniation clear as daylight so that one is going. But I have started to get pain in the middle of my neck my muscles in my jaw are tight and pop. the back of my throat feels like a sorethroat from all the muscles spasming. So Dr Lauryssen I saw was nice. He treats the stars. paula Abdul and Phyllis Diller. But he was rather radical stating that I should have a 3 level with of course prodisc in US. Then Dr Fenk-meyer from Dr Bertagnoli agreed with that. Then in US local surgeons said just c6-7. I guess leaving me in partial pain???
Then Dr Boeree thought c 5-6 is normal and would do 2 levels that are NOT adjacent--c4-5 and c6-7. If he did that THERE WOULD BE 2 SEPARATE INCISIONS---OH HOW FUN!!!!! :welcome:

ANYONE HAVE thoughts about how one decides or clarifies this mess. My pain is enormous. I have stopped most all activities and wear a brace 24-7 to keep the pain to a minimum. I have turned couch potato into a new meaning  PS sorry for the additional post tha tis repetitive!

Deciding on surgery with Dr Boeree

Dear Jeff http://www.spinepatientsociety.org/forum/images/smilies/welcome.gif
I was thinking about you. So glad you are doing so well--such an inspiration. I have finally decided to get the surgery given my recent shall I say pain in the neck http://www.spinepatientsociety.org/forum/images/smilies/default/frown.gif
I have the usual DDD. I have a herniation at c6-7 which is clear. But then a bulge at c4-5 which some think is causing pain higher in my neck and jaw. Did you have such symptoms? How did Dr Clavel decide to do the segments he did on you? Was it clear from symptoms and MRI or did you have other testing?

I have had many different opinions--the more radical approaches say 3 levels c4-5 through c6-7. And in US they are cautious and say 1 level c6-7. Dr Boeree said m-6 on c4-5 and c 6-7. Which leaves c5-6 right in the middle of a sandwich since he thinks c 5-6 is ok.

i know you have a different situation since your fusion is in the middle of the ADR's.
But I wondered how you decided on the m6. I was told by Dr Boeree that it is the preferred ADR in Europe now. You decided on this I'm thinking years ago when it was recently available. The worry I have is how much pressure it puts on the facet joints and if putting c5-6 in the middle it would somehow impact that level. i know I could have a discogram but I was told those are not terribly accurate?? Well anyway don't mean to bother you but really would value your thoughts!!
