I am usually pretty good at reading MRI's, but I cannot for the life of me figure out what this is? It (attached) comes from a axial view of my back. I put a arrow showing what I am talking about, the big black round part towards the upper right of the pic. Any body can tell me for sure?

- 1997 Injured back at work, diagnosed as bulging disc at L5-S1
- Years of PT and different non-opioids
- 04/2003 Herniated disc
- Many epidural injections, facet injection, SI injection, no success
- 10/2003 Nucleoplasty L5-S1 (failed)
- Tried different opioids; now on oxycodone
- 2005 Moved from NJ to NC
- 2008 Volunteer at local community health center (love it!).
- Hurt back again, found great ADR doc in NC, accepted into Freedom Disc clinical trial
- Will have ADR end of Jan 2011

L4-L5 Annular fissure with mild central stenosis and moderate facet hypertrophy.
L5-S1 bilaterial neural foraminal narrowing with inferior effacement.
L2-L3 Right-sided neural foraminal narrowing
L3-L4 related to posterolateral hypertrophic spurs and facet hypertrophy.
C3-C4 limited DDD

Ken,
Your MRIs conclusion is that you have minimal degenerative changes at L5/S1 with a mild disc bulge posteriorly. There is also mild foraminal stenosis bilaterally at L3/L4. Also noted on the report is an incidental cyst on the left kidney.

As far as the renal cyst goes quite a few have them. They are graded on complexity with anything over a three on the Bosnic scale needs a ultrasound or CT, say yearly. I'm not that smart, I just have one too. Had it for 20+ years and has always been a Bosnic II the whole time. I never know it is there. It is one of those not cancers but on occasion does step over the line. With a Bosnic II, on source said as many 16 % can have renal cancer just from this level. Most kidney cancers are renal cell type and rarely metastasize distally, but will over time grow out and invade. Bosniak renal cyst classification | Article | Radiopaedia.org

Low back pain became somewhat dehabilitating in 2005
Have had 11 steroid injections, IDET, Trial for nerve stimulator, PT, chiropractic trial, practically every med known to mankind. Discogram indicated three diseased levels with L5-S1 being the most likely pain generator. Post minimally invasive PLIF with internal fixation (titanium) on 12-28-09 of L5-S1. Doing better than expected. Last opioid 7/9/10. Five months pain free, then my neck turned against me. MRI on 12/1/10-- disease at C2 to C7. Only surgical alternative is to fuse entire C-spine. Diagnosed now with Aggressive Relapsing-Remitting Multiple Sclerosis with cord & brainstem active lesions
