Welcome to the site. I'm sorry you are having so much trouble. I'm having a lot of the same problems as you, especially the bat in the back!
I also have United and as far as I know they do not cover ADR according to my doctor. I haven't tried to contact them. I am getting reenrolled in a clinical trial next Tues. for the Axiomed/Prodisc. It is for one level only but I've heard Justin and Katie say that some places do sympathy cases or something like that. Do you need both L4 and L5? If I decide to do this surgery I will have a 50/50 chance of getting the Freedom Disc or the Prodisc. I will also have to pay my max out of pocket from United which is $5,000 which I really don't have. They will follow my progress for several years and I will have to get a lot of X-rays.
