I flew coach for both of my surgeries in Germany. I didn't have a problem on the flight and I saved a ton of money on each flight. Just inform the staff that you will be getting up periodically to walk around (from your seat to the back of the plane, etc.).
Also, it was mentioned to have a wheelchair set up in advance for your return flight. This is key. It will allow you to pre-board the airplane and you will absolutely appreciate not walking the football-like field distances to your gate. It is also a good idea to check your bags curbside and have the driver help lift your bags.
Hooch, you can also have your surgeon type a letter describing your operation and if the airline can accommodate you with seating, etc. This hardly works nowadays, but it might be helpful. You never know.

Beg, borrow or buy airline miles from friends or family and book a business class seat at the least for the flight home. Many of the airlines have business class seats that lie flat now.
