From what you're saying on the billing that you've received, your percentage is being paid off the negotiated price the doctor/hosptial has with your insurance company. You should receive in the mail some type of Explanation of Benefits (EOB) that explains what was billed, what the insurance company will pay (This amount becomes the total cost as far as your concerned), your copayment (if any), your deductible (if any), and any amount you have to pay (this would be your 30%). All that should add up to the allowed payment. I know for me, when I got my bill all that was listed on the Explanation of Benefits. For me, I thought I was going to have to pay a percentage based on my plan, but it ended up being covered 100%. There must be some obscure clause in my plan that I missed.
The EOB should make it fairly easy to figure things out. If you're not sure look at the EOB and call your insurance company. They'll be able to answer your questions about billing based on what your policy covers.

1976 - Fell from tree. The beginning!
1985 - Car accident - pull back muscle.
1985 - Spring 2010 -Flare ups off and on. Meds manage - Life goes on.
9/2010 -Pain returns, Diag DDD of the L4/5 & L5/S1 space with arthritis. .
10/ 2010 - Referral to PM Doctor & PT. PT not helpful. MRI confirms DDD, bulging disc of L4/5space. Pneumonia. Pain off charts!
11/2010 - 3/ 2011 2 RSNB/2 ESI. Relief short lived .Referred to surgeon.
6/29/11 -TLIF Fusion of L4/5 space.

43 yr. old female with 11 and 13 year old kids.
10+ years of chronic back pain (Severe R/L leg pain for past year)
DDD at L3L4, L4L5, L5S1
Herniations, foraminal stenosis and facet hypertrophy at all 3 levels
Type II modic changes at L5S1
Conservative measures not helpful (medication, NSAIDs, injections, PT, massage, chiro, lifestyle and ergonomic modifications)
Microdiscectomy/Laminotomy (L5S1) 12/10 - Reherniated 6 weeks later.
2-level fusion (ALIF) (L4-L5, L5-S1) 6/29/11
Hi Bob.
How are you feeling now? I am very interested to see how you feel at this point after the surgery. I had my hardware removal surgery on 11/30. It's still hard to tell if I feel better, but I'm hopeful.

- 2008 Discogram, bone scan, MRI, and numerous CT scans
- 2008- pars defect congenital, Spondylolisthesis grade 2

- 2011: Another bone scan revealed bone loss in Spine. Diagnosed with Osteopenia
- 2011: Spinal hardware removal surgery on 11/30
